i don't think so .i think bowel perforation and rupture viscous that are differential diagnosis it may be chilaiditi syndrome cxr shows the presence of the hepatic flexure of the colon interposed between the liver and diaphragm(chilaiditi's sign) chilaiditi syndome is this fiding plus symptomatology: nause,vomiting .abdominal pain, anorexia,constipation or respiratory distress
:) I propose another scenario : that guy have lung cancer (maybe epidermoid cancer in the right middle lobe), and because of treatment of chemotherapy, he has abdominal pain, nausea and he is vomiting.
if I extend my opinion, some lung cancer may extend and cause paralysis on the phrenic nerve causing ascension of the hemi-diaphragm, as can be seen on the Rx (right hemi-diaphragm)
