I'm no radiologist, but let's see. It's in between the hyoid and the vertebra, it's a fairly large tube, and it's got a mishapped flap. Definitely a case of acute epiglottitis. If the patient presents fever, it's caused by a bacterial infection; take a blood culture to confirm. If I'm right, E is pointing to an air pouch/column, while A is the site of inflammation.
